This module aims to further develop practitioners knowledge and skills in the use of the NHS Health Check programme to facilitate the cardiovascular assessment of the eligible population. It gives practical guidance to health care practitioners, (e.g. health care assistants, support workers, health trainers, pharmacists, nurses, private providers) who will be involved in the delivery of the NHS Health Check.  The module enhances knowledge and skills to deliver a cardiovascular risk assessment to the eligible population in England between the ages of 40-74 years without existing cardiovascular disease.

The NHS Health Check is explained as a systematic process from eligibility criteria, risk assessment, identification of an individual’s chances of developing cardiovascular disease, diabetes and kidney disease, communication risk, lifestyle interventions and referral to relevant health care professional.
